"Safe Sanctuary" Forms

In our efforts to keep our children and youth safe while they are under our care, Lititz United Methodist Church requires that everyone over the age of 18 who works with children or youth have current police and child abuse clearances on file.  Our conference has ruled that these clearances are valid for three years, and then must be renewed.   If you need to initiate or renew your clearances, please print and complete all three of these documents and return them to Lynn McCord in the church office.  In order to help offset the costs for the church, we request that a $20 donation be given to pay for the clearance checks.  If you have any questions, please contact Lynn McCord.
